February 1 Current Family, Re-Enrollment
February 18 Open Enrollment

2. Provide Necessary Documentation
Emergency Information and Immunization Record Card
Staff, Student and Parent Commitment
Parent Participation Form
Financial Contract
Signed/Parent Student Handbook Agreement
Picture/TV/Media Authorization Release Form
Copy of Updated Immunization Card
3. Yearly Pastoral Recommendation Form
